Upload Your COVID-19 Vaccine Information Early

We are looking forward to seeing you back on campus for Fall 2021. As outlined in our plan for safe re-entry, all students and employees returning to campus will be tested for the virus that causes COVID-19 at least weekly. Those who choose to be vaccinated can opt out of the testing plan by verifying that they have been fully vaccinated. In addition, as recently announced, the California State University will require all employees and students to be fully immunized against COVID-19 prior to returning to any CSU campus after the U.S. Food and Drug Administration grants full approval to at least one COVID-19 vaccine and the vaccine(s) are sufficiently available.
In anticipation of this return to campus, Cal State Fullerton encourages students to voluntarily upload their COVID-19 vaccination verification information now. Students can easily upload their vaccine verification information online, which follows similar HIPAA (referred as Health Insurance Portability and Accountability Act of 1996) protected methods for other campus required immunizations. If you are currently a student employee on campus, please do not upload your vaccine information until further guidance is emailed to you from Human Resources, Diversity & Inclusion (HRDI).

Your privacy is important to us. Your COVID-19 vaccination status will be used to exempt you from weekly testing; it will not be shared with others.
There are several benefits to uploading your vaccination information early. Most importantly, completing the verification process eases your re-entry to campus for the Fall 2021 semester. In addition, you will be able to participate in any pre-Fall 2021 semester activities on campus.
Thank you for your patience and flexibility as we navigate these changes together. Once finalized, we will communicate any additional updates regarding the California State University vaccine guidelines such as medical and religious exemption information.
